Patient Registration

If you live within the Practice boundary and have a medical card, simply bring it to the surgery and hand it to the receptionist, having filled in your new address and contact telephone number.  If you have mislaid it, we can provide you with the necessary form to complete. Medical cards are no longer issued by local Health Authorities, so do not be worried if you do not receive a new card through the post.

All new patients are requested to see one of the practice nurses for a general health check.
