Please inform the Practice immediately if you change your name, telephone number or move to a new address. If you wish to change your name, please bring a copy of any relevant documents, eg, marriage certificate or deed poll paperwork. To do this please click on the Online Services link on the home page or complete a change of details form at Reception.
It is essential that we keep our records up to date in case of an emergency or Hospital referral.
If you move to an address that is outside our Practice boundary, you may be asked to re-register with a General Practitioner who covers that area.
